The balloon factory blow up due to something that technically was her fault and she lost the resulting lawsuit.
How did she cause the factory to explode?

Her job entailed filling an entire room with baloons then popping them one by one. Having gone out drinking the night before, she came into work one day tired and with a hangover and accidently filled all her baloons with hydrogen rather than helium. When she popped her first baloon, it exploded and caused a chain reaction to all the other baloons in the room.
How did she survive the incident?

She didn't, she became a ghost. On the plus side she no longer has to worry about paying for food and water.
What are some of the downsides of being a ghost?
